A Guide to Moving from One Care Home to Another

There are many reasons why someone may consider moving from one care home to another. Whether their care needs have changed or they are looking to relocate, sometimes changing care homes is the best option. Finding a residence that is a better fit may make a big difference to an individual’s quality of life, health, wellbeing and contentment.

Moving from one care home to another isn't always quick or an easy decision to make. However, there are a number of circumstances in which someone may need to find a new residence:

• Their care needs have changed and can no longer be met – for example, they need to move to a home that offers nursing care
• They are unhappy with the standard of care, food, facilities or activities on offer
• Their financial situation has changed and they can no longer afford the care home fees
• They wish to relocate to a different area of the country – for example, to be closer to family

Changing care homes is a big decision, so it's important to carefully consider your options. If you have an issue with the standard of care, for example, it may be worth talking to the care home manager first. You may find that you can work together to resolve the problem. You may also wish to talk things through with a loved one, friend or advocate and request their guidance and support.

Step-by-Step Guide to Changing Care Homes

If you have decided on moving nursing home (or residential home) for yourself or a loved one, you may wish to follow these steps to help make the process as smooth as possible.

1. Review the Contract

First, look over the contract between the resident and the care home. This will state how much notice they need to give and any other terms relating to leaving the property.
In some cases, an exit penalty may be payable for ending the contract early. Ask a member of staff if there are any terms or conditions that you don't understand.
If your local authority covers the care home fees, the contract will be between the care home and their social services. You will need to contact your local council to move things forward.

2. Book a Needs Assessment

If the resident receives funding from their local authority, and the current home cannot meet their needs, you should schedule a needs assessment. This is usually not necessary when self-funding.

The assessment will determine the level of care the person requires, and whether they would benefit from moving homes. If so, they will normally be given a list of suitable residences to choose from.

However, you have the right to choose a different home, provided it can meet the individual’s needs. This may be necessary if moving to a care home in a different area.

3. Find a New Care Home

Before you decide to give notice for leaving a care home, you can begin searching for a new residence. A good place to start is the Care Quality Commission (CQC). The CQC is the independent regulator of health and social care in England. They publish contact details and inspection reports for all care homes in the country. You can use the CQC website to filter residences according to:

• Location
• Type of home (residential or nursing)
• Speciality (e.g. dementia)
• Inspection rating

If nursing care is required, you'll need to find a nursing home or a dual-registered home (such as KYN). Dual-registered homes can accommodate both residential and nursing needs.

4. Arrange a Visit

Before moving nursing homes or care homes, it's essential to visit the property in person. This will give you and your loved ones a chance to experience the accommodation and facilities for yourselves. During your visit, you should ask as many questions as possible, such as:
• What training and qualifications do the carers have and how many carers will be on duty at any given time?
• What kinds of activities, enrichment and entertainment are on offer?
• What type of food is served and is there a choice in dining environments? Can the chefs cater to dietary requirements and preferences?
• Are the residents happy with the care they receive?
• What is included (and not included) in the fees? For example, are the bedrooms furnished?
• Are friends and family free to visit whenever they like?
• Has the service been inspected by CQC and what is the rating?

You can also request a brochure or guide regarding the service that you can take with you and study more closely. If you feel that the home is a good fit, you can ask to reserve a place.

5. Decide on a Moving Date

The final step is to arrange a moving date. This may depend on when the room will be ready, and the notice period required by the person’s current home.

The current care home or local authority may be able to facilitate the move, including arranging transport for personal possessions. However, you may have to make your own arrangements, particularly if self-funding.

Talk to the care home staff to begin organising the move. The carers will explain what happens when moving someone out of a care home, and how they can help with a stress-free transition.
